Paperwork – New Client Forms and Consent Forms

Q: Why do I have to complete paperwork prior to my appointment?

A: Your safety and comfort are our primary concerns. We require disclosure of all medications, medical conditions, allergies, and other important information prior to your first service. In addition, we may ask you to update your info from time to time to disclose any new medical conditions or medications. We use these forms to create a safe and effective service for you. If we see any contraindications to service on your forms, we will contact you to discuss alternative services. It is imperative that you be 100% honest and thorough in your answers.

 

Waxing

Q: What kind of wax do you use?

A: We primarily use hard wax for intimate areas and soft (strip) wax or roll on wax for arms, legs, chest, face, and back. Depending on your specific hair type and its location, we may use either or both types. We strive to cater to our sensitive skin clients by offering hypoallergenic and low/no scent wax when available. Also, we use assorted cleansing products, oils, and powders in waxing services.

 

Q: Will waxing be painful?

A: Pain tolerance is different for each person and in each area of the body. Generally, you should expect some mild to moderate discomfort. After all, we’re removing hair from the follicle. Scheduling regular waxing services will help build your confidence in the process and get most of your hair on the same growth cycle.

 

Q: Can I bring someone into the treatment room with me?

A: Parents of minor children may accompany their child into the treatment room. Otherwise, no guests allowed.

 

Intimate Area Waxing

Q: Do you offer waxing services for men?

A: Yes. However, we do not offer male genitalia intimate area waxing (no Brazilian, bikini, or bum waxing).

 

Q: Can I get a bikini or Brazilian wax while on my period?

A: Yes. Please shower immediately before your appointment and wear a tampon or menstrual cup.

 

Q: How long does my hair need to be?

A: A minimum of ¼” long or about the length of a grain of rice. Any shorter and the wax will not remove the hairs.

 

Q: There are still a few hairs left in the waxed area. What now?

A: Hair has a growth cycle and grows at different speeds in each cycle. We cannot guarantee that every hair will be removed due to different lengths, thicknesses, etc. Also, hairs may break off rather than be entirely pulled out from the follicle. We may tweeze a few hairs during your service to remove some stragglers. Scheduling regular waxing will help you to get hairs on a similar growth cycle which can improve your waxing results. Also, waiting until your hair is a minimum ¼” length will help the process.

 

Q: I’ve never had an intimate waxing or it’s been a long time. What can I expect?

A: Our goal is to make your experience as comfortable as possible. We are not speed waxers, meaning we won’t rush you in and out. We will observe the area to be waxed prior to starting the service and, if necessary, offer trimming first.

There are a few things you can do to prepare for your appointment:

 *Shower immediately before

 *Take an over-the-counter pain reliever like Tylenol or ibuprofen

  *If you are on your period, please wear a tampon or menstrual cup

            

Q: I’m pregnant. Can I get a Brazilian wax?

A: Yes. We offer waxing until 24 weeks gestation only. During pregnancy, waxing can be more painful. Your skin may swell due to increased blood flow and become very sensitive, and your hair may become thicker. You’re also more susceptible to infection. We recommend waiting until you are post-partum and have clearance from your medical provider to resume waxing. Please note: If you schedule a waxing but “tap out” during the service due to pain or discomfort, you will still be charged the entire fee for the service.

 

Facials

Q: What facials do you offer?

A: When you book a facial at Warm Cedar, you are treated as an individual with unique skin care goals. While all of our facials include basic steps such as cleansing and exfoliating, we customize techniques, products, and ingredients specifically designed to address your skin’s current condition. When you schedule your appointment, we will email a form to you to complete at least 12 hours prior to your service. We will ask about your skin condition, allergies, products you use, medications you take, procedures you’ve had, and other info that will help us to build a facial specifically for you. Complete this form with 100% honesty so we can check for any contraindications. If there are any areas of concern, we will contact you promptly to discuss options and alternatives. We will not provide services to any client who has not completed new client form. It is for your safety and comfort. Also, these forms will be kept in a confidential way.

Q: Do you offer chemical peels?

A: Yes. In order or us to get to know your skin and choose the correct peel or set of peels for you, we require you schedule a basic facial at least one month prior to any chemical peel service.

Q: Do you guarantee results?

A: Short answer: No.

Long answer: Skin care is a lifelong journey. What we do during a facial is a small percentage of the trip. The rest is up to you by way of consistent home care, wearing sunscreen every day, avoiding smoking, avoiding tanning, eating healthy, and staying hydrated. Many internal and external factors contribute to your skin’s condition. Scheduling monthly facials is another step you can take so we can track your progress and make adjustments to your routine.

 

Q: What if I have a reaction to a product or service?

A: During your service, we will monitor your reactions closely, removing any products immediately and taking necessary steps to minimize the reaction. If you have a medical condition that requires the use of an EpiPen or similar devise, please tell us prior to your service. After the service, if you notice an immediate, life-threatening reaction, please contact a medical professional immediately or dial 911. If you have a reaction up to 2 days after your service, please consult a medical professional. Then, contact us so we can note the reaction in your file.

 

Q: I’ve had Botox, dermal fillers, Restylane, cosmetic surgery, or other cosmetic procedures. Can I still schedule a facial / microdermabrasion / dermaplaning / chemical peel?

A: If you have had Botox, fillers or any other injectable cosmetic procedure, you’ll need to wait a minimum of 2 weeks after your last treatment to have a facial. If you’ve had any surgical procedure, we require written clearance from your medical provider listing the date after which you are cleared for facial services. We may require written clearance if at any time we have concerns about a service.

 

Height and Weight Considerations

Q: What are the height and weight considerations on your spa equipment?

A: Our spa beds have a manufacturer maximum weight of 400 pounds and can comfortably accommodate a height of up to 6’. However, services such as brow waxing may be performed seated or standing. We strive to be inclusive of all body shapes and all are welcome. Please contact us if you have questions or concerns.

 

Payment and Refunds

Q: What forms of payment do you take?

A: All major credit cards, cash, gift cards purchased through our website or in person. Credit card payments are processed through Square. No Apple Pay. No Venmo. Invoices must be paid in full at the conclusion of the appointment. The credit card info you enter at the time of booking may not be kept on file, so please bring a form of payment with you to your service. Failure to pay will prohibit you from scheduling in the future and may result in a referral to a collection agency. Theft of service will be reported to the local police.

 

Q: Do you require a deposit?

A: Deposits may be required to reserve your appointment as a form of no-show protection. The standard no-call/no-show fee is $15 or a percentage of your service total, whichever is greater. Also, if you repeatedly cancel or reschedule appointments, we may require a deposit to secure future services. Please see our cancellation and reschedule policy for more info.

 

Q: Do you offer refunds?

A: No refunds for any service.

 

Minimum Age for Services

Q: What is the minimum age for services?

A: We offer basic facials, acne facials, and back acne facials for minors ages 12 and above. You must be age 18 or older for all other services. All clients under the age of 18 require a signed waiver from a parent or legal guardian prior to service as well as a completed liability waiver and client info form.

Q: Can I schedule a service for someone else?

A: No, unless that person is your minor child.

 

Q: Do you ever refuse service?

A: Warm Cedar Esthetics, LLC reserves the right to refuse service to any person at any time. Any behavior deemed inappropriate will be grounds for immediate conclusion of the service. The client will be asked to leave, blocked from re-booking any services, and will be charged in full for the current service(s).
